New Horizons: Transitional Living for Homeless Veterans in Hebron, IL

To help clients regain their ability to live independently, New Horizons provides resident-specific case management that emphasizes self determination—with the resident and case manager collaborating to determine a resident’s specific needs and goals as they develop a care plan. A three-track approach to guide struggling veterans back to a fulfilling and dignified life is employed.
- Track 1, Residental Stability:
New Horizons will guide homeless residents toward residential stability by helping them acquire basic life skills, addressing issues of chronic underemployment and unemployment and focusing on their responsibility for their mental and physical well being. Within a month of enrolling, each resident will begin development of care plan that outlines in detail each step necessary to move from transitional living to permanent housing.

- Track 2, Improved Skills:
New Horizons will promote improved life skills by providing or referring residents to financial counseling, on-site group sessions, relapse prevention and community-based Alcoholic Anonymous meetings. All unemployed clients will be referred to Illinois Department of Employment Security for resume development and access to job search tools.

- Track 3, Greater Self-Determination:
New Horizons will move veterans toward greater self-determination by encouraging them to plan for the future and be active participants in their care plan. Residents will meet with a case manager within a month to establish a list of their individual goals and strategies for achieving their goals. A goal could be a data entry job. The strategy could be to learn a spreadsheet application and then arranging transportation to the public library for a free tutorial. Within 60 days of establishing their goals, residents will assess progress made toward goals and if applicable set new goals.
New Horizons strives to return veterans to residential stability within in nine months, but residents can stay at the center for up to two years if needed.
All residents are strongly encouraged to participate in an aftercare program after they move on to independent living. TLS remains committed to each of its clients even after they are living on their own and will continue to provide services until it is determined the client is able to maintain a stable life style.
